Fill up all the details, e.g. Your name, Company, License serial number, and the reason need to reset, sign with company stamp.

SUBMIT

Rescan the form and summit online to us.

APPROVALS

We will notify you by email when the reset is approved, Portal user able to check the status from My Account / Ticket page.

For your reference, we provide complete archives and history of all your support requests.

A valid email address is required to submit a ticket.
